Medical Device GMP: How the Four New Inspection Principles Judge Conformity | 医疗器械 GMP:四个新检查指导原则如何判定符合性

Overview

On 23 October 2026 the National Medical Products Administration issued four inspection principles to accompany the revised Medical Device Good Manufacturing Practice, which takes effect on 1 November 2026. The four cover sterile medical devices, implantable medical devices, in vitro diagnostic reagents and customised dentures, and they take effect on the same day as the revised GMP.

The notice also repeals what came before: the 2015 on-site inspection principles for sterile, implantable and IVD production (Shi Yao Jian Xie Jian [2015] No. 218) and the 2016 principles for customised dentures (Shi Yao Jian Xie Jian [2016] No. 165).

Why the annexes were folded in

The drafting method is stated plainly. Each principle takes the GMP and the corresponding annex — sterile, implantable and so on — merges them, and converts the combined text into inspection items. The annex stops being a separate document read beside the GMP; it becomes part of the checklist.

That changes how a quality team holds its documentation. A system that mapped its procedures to the GMP and treated the annex as a second layer now has one layer, and the mapping must be rebuilt rather than relabelled.

Who is measured on the new scale

Four product families: sterile devices, implantable devices, IVD reagents and customised dentures. Both production licence verification — new applications and scope changes — and post-market supervision inspections use the new principles to determine results.

That second channel is the one companies underestimate. A licence holder that passed verification years ago is not exempt; the next supervision inspection applies the same new scale. Contract manufacturers sit inside the same perimeter, because the rules attach to the production activity rather than to the brand on the label, and a customer audit does not substitute for the regulator’s.

For a foreign group, the exposure sits in Chinese plants making these four families, whether wholly owned or in a joint venture. A plant outside them is untouched by the four principles but still bound by the GMP itself.

What the three grades do

The notice states that items are graded into critical, major and general according to each clause’s importance in the quality system and the risk created when it is not met. The grading is a risk statement rather than a scoring system: it tells an inspector which findings can be tolerated and which cannot.

One point needs care. Practice treats a failed critical item as a failed inspection. That is the settled approach in GMP inspection, not a sentence in these four documents, and it should be attributed to practice rather than to the notice.

Preparing for an inspection under the new scale

A supervision inspection that arrives unannounced is the wrong moment to discover that the quality manual still cites a repealed on-site principle. Three artefacts are worth assembling before the inspection rather than during it.

The first is a clause map: for each critical item in the merged checklist, the internal procedure that satisfies it, its version date, and the person accountable. A map that cannot be produced within a day is not finished, because it is the document an inspector will ask for first.

The second is a change log for the two years before the inspection, covering process, supplier and equipment changes in the four families. A sterilisation parameter altered without revalidation reads as an unvalidated process, not as an administrative gap.

The third is the contract manufacturing chain. Where a foreign group uses a Chinese contract manufacturer, the group’s own quality agreement should already allocate responsibility for the merged items; if the agreement predates the merged checklist, it allocates responsibility for a document that no longer exists.

None of this is a legal requirement in the notice. It is the working order in which inspectors tend to ask, and preparing in that order shortens the inspection rather than the requirements.

Where the four lines differ

The four principles share a structure but not a weak point. For sterile devices the recurring failures cluster around cleanroom environment monitoring, sterilisation process validation and sterile barrier packaging validation. For implantable devices the pressure falls on traceability, from raw material to the finished device. For IVD reagents it falls on raw material and calibrator management, where a supplier change can move a result. For customised dentures it falls on the patient model and on material traceability, because each unit is made to an individual.

These are listed by product logic, not by clause number; the documents are the authority on which item sits in which grade.

A practical timetable

  • Complete a gap analysis now, not at renewal; the new scale already applies.
  • Convert the quality manual and any procedure citing the annex, then re-map to the merged items.
  • Run an internal audit using the three-grade logic, so that findings are ranked the way an inspector will rank them.
  • Re-evaluate suppliers whose materials feed a critical item, especially IVD raw and implantable materials.
  • Keep the annual self-inspection for products already licensed, and record it against the new item structure.

What this does not change

This is a production-end instrument. It does not alter registration or filing requirements, does not open a new channel and does not change how a device is cleared for the market. A registered device needs no new filing; what changes is how its factory is inspected.

Teams that read the four principles as a registration document will spend a quarter on the wrong file; teams that track registration only meet the risk on inspection day.

Outlook

Expect inspection reports to cite merged clauses rather than annex paragraphs, and expect internal document maps that still separate the GMP from its annexes to look dated within a year. The substantive requirements have not been rewritten; what changed is where they are written down and how findings are graded.

The practical test of readiness is simple: ask the quality head for the clause behind each critical item. If the answer is a page in an annex that no longer stands alone, the conversion has not been done.
