Healthcare Access & Medical Insurance for Expatriates in China | 外籍人士在华就医与医疗保险指南
Overview
For a foreign national arriving in China — whether as an executive, an engineer on assignment, or a trailing spouse — healthcare is usually the first practical worry after housing. China’s system is different from many home countries: it is a statutory social-insurance backbone plus a fast-growing commercial-insurance market, layered over a hospital network that mixes public tertiary hospitals, international departments and private clinics. This article maps how an expatriate actually gets care and pays for it, what the law requires of employers, and which 2026 measures make daily life easier. Who is covered — the statutory basis
The backbone is the Social Insurance Law of the PRC and the Interim Measures for Foreigners Employed in China to Participate in Social Insurance (Ministry of Human Resources and Social Security Order No. 16, amended 2024). Under Article 3, employers registered in China that hire foreigners must enroll them in five insurances: basic old-age, basic medical, work injury, unemployment and maternity — with premiums shared by employer and employee. Article 4 requires registration within 30 days of the work permit. Foreigners dispatched from an overseas employer to a China branch are covered the same way by the in-China work unit.
For unemployed permanent-residence holders, the Notice on Treatment Enjoyed by Foreigners with Permanent Residence in China (MOHRSS Fa [2012] No. 53) lets them use the Permanent Resident ID as a valid ID to enrol in social insurance; those not employed but meeting local conditions may join urban-rural resident basic medical insurance. Foreign students enrol under school-location rules.
How to enroll
- Employed foreigners: the employer handles it. After the work permit and residence permit are issued, HR opens social-insurance registration at the local social-insurance/medical-security service hall or online portal; the employee then receives a 医保电子凭证 (electronic medical-insurance credential) usable via the 国家医保服务平台 app.
- Unemployed Green-Card holders: apply at the local medical-security agency with the Permanent Resident ID; choose resident basic medical insurance.
- Newborns/students: foreign newborns born in China and foreign students may enrol in resident basic medical insurance per local rules — several cities (e.g. Jiangmen) have publicly reported cases of foreign families reimbursed through the system.
Commercial & private insurance
Because public coverage alone may not meet expatriate expectations (waiting times, language, network hospitals), most foreign employees and their families also hold commercial international health insurance. Products range from 惠民保 and 百万医疗险 to mid- and high-end medical insurance (高端医疗险). High-end plans — often the default choice for expats — typically cover private hospitals and international departments of public hospitals, offer direct billing (直付), multilingual support, 24/7 service and emergency medical evacuation. Many carriers (incl. Allianz, AIA, Cigna & CMB, Generali China) underwrite foreigners; most require a minimum stay (commonly six months+) and passport-based application. A few city 惠民保 schemes now admit foreigners who have joined social insurance.
Using the healthcare system day-to-day
- Public tertiary hospitals: book via 公众号/APP or on-site; bring passport + residence permit + 医保电子凭证. Expect Chinese-language service; international departments (国际医疗部) exist in major cities.
- Private & international clinics: common in Beijing, Shanghai, Guangzhou, Shenzhen; usually English-friendly, higher cost, often direct-billed by insurer.
- Emergency: dial 120 for ambulance. Keep a note of blood type, allergies and your insurer’s emergency line.
- Cross-border (Greater Bay Area): several insurers now offer GBA cross-border medical plans linking mainland, Hong Kong and Macao networks.

谁被覆盖——法律依据
骨干是《中华人民共和国社会保险法》与《在中国境内就业的外国人参加社会保险暂行办法》(人力资源社会保障部令第 16 号,2024 年修正)。其第三条规定,在中国境内依法注册登记的用人单位招用外国人,必须为其参加基本养老保险、基本医疗保险、工伤保险、失业保险、生育保险五项,保费由单位和本人按规定缴纳;第四条要求自办理就业证件之日起 30 日内办理社会保险登记。受境外雇主派遣到境内分支机构的,由境内工作单位同样参保。
对未就业的永久居留持有人,《外国人在中国永久居留享有相关待遇的办法》(人社部发〔2012〕53 号)允许以《外国人永久居留证》作为有效身份证件办理参保手续;在境内居住但未就业、且符合统筹地区规定的,可参照参加城乡居民基本医疗保险。外籍留学生按学校所在地规定参保。
如何参保
- 就业外国人:由单位办理。工作许可与居留证件办妥后,HR 在属地社保/医保经办大厅或网上平台开立社保登记,个人随后领取可在”国家医保服务平台”App 使用的医保电子凭证。
- 未就业绿卡持有人:凭永久居留身份证在属地医保经办机构申请,选择城乡居民基本医疗保险。
- 新生儿/学生:在华出生的外籍新生儿与外籍学生可按当地规定参加城乡居民基本医疗保险——已有城市(如江门)公开报道外籍家庭通过该系统获得报销的案例。
商业与高端医疗保险
由于仅靠公共保障未必满足外籍人士对候诊、语言与网络医院的要求,多数外籍员工及家属还会投保商业国际健康险。产品从惠民保、百万医疗险,到中端、高端医疗险(高端医疗险)不等。高端计划往往是外籍人士的默认选择,通常覆盖私立医院与公立医院国际部,提供直付(直付)、多语支持、7×24 服务与紧急医疗转运。多家险企(含安联、友邦、招商信诺、中意等)承保外籍人士;多数要求最短居住时长(常见半年及以上)并以护照投保。少数城市惠民保已对参加本地社保的外籍人士开放。
